Extended Care Model
Benefit from unwavering care and stability, with an indefinite length of stay tailored to individual needs.
Our extended care model offers 24 hour support and supervision in a residential setting. Each resident will work with their treatment team to develop an individualized treatment plan. Our minimum length of stay is 6 months with an average length of stay of 12-30 years.

Our Supported Community Living program allows residents to live and work independently in the community while remaining closely connected to Lakewood Center for support when needed. Designed with a community integration philosophy, this level of care empowers individuals to build a fulfilling life outside of a residential setting while still receiving wraparound services to help them thrive. With flexible care tailored to personal goals, residents benefit from ongoing connection and support, fostering independence without the need for continuous supervision.

The Lakewood Therapeutic Work Program is designed to empower our residents by providing meaningful, paid employment opportunities either on campus or within the Lakewood community. Through structured job placements, participants develop essential job skills, enhancing their confidence, competence, and independence. The program is built on the principles of personalized support, offering guidance tailored to each individual’s strengths, interests, and mental health needs.
